Do I need a physician referral to schedule an appointment?

The requirements of your health insurance plan will determine if you need to obtain a physician referral. Our staff may be able to determine this over the phone, but if you have any questions, it is best to contact your insurance carrier for an answer.

What do I bring for my appointment?

For your first visit, please bring your insurance card, your driver’s license or photo ID, the authorization form from your primary care physician (, if required by your insurance plan), any chest x-ray, CT scan, or other relevant tests you may have had previously done, your current medication list, and the completed forms from the New Patient Packet if you have not completed the forms electronically.

Why do I need to update my medication list at each visit?

We need to be aware of the medications that you are currently taking in order to thoroughly evaluate your condition. This evaluation process includes reviewing all your medications, including drugs prescribed by your primary physician and any other specialist.​

Will my primary care physician be informed of the results of my consultation?

Your primary and referring physician(s) will receive a letter detailing the results and recommendations from your consultation, including the results of any relevant pulmonary function testing.​

What is our cancellation policy?

We require 24-hour notice for cancellation. In the event that you do not cancel your scheduled appointment within 24 hours, there will be a $125 fee for new patients and a $35.00 fee for established patients.

What if I am running late for my appointment?

Please call the office to let us know if you will be running late and give us an estimate of your arrival time. We will make every effort to honor your appointment, but in some cases you may need to wait to be seen or reschedule your appointment.

What if I need a prescription refill?

Please call your pharmacy and request that they fax a refill authorization to our office. Unfortunately, we are only able to authorize refills on medications prescribed by our physicians. We make every effort to honor refill requests within 24 hours.​

Are my medical records kept private and confidential?

Your medical file is handled with the utmost respect for your privacy. Our staff members are bound by strict confidentiality requirements as a condition of employment regarding your medical records.  Please review our Notice of Privacy Practices.
